The problem

A £5M customer portal programme was failing. Testing was behind, defects were mounting, and contractual penalty clauses were weeks from triggering. The delivery team knew it. Morale had collapsed into blame.

The intervention

I merged the development and QA units into one delivery organism with a single definition of done. Testing shifted to daily, risk-based prioritisation: highest-consequence paths first, not coverage theatre. And the leadership style shifted from blame to leader-led coaching: the team that broke it became the team that fixed it.

The result
